This is not factory made - it was built by a carnival concessionaire for use on a traveling midway and has 70+ years of honest carnival wear.

This is the classic"Tip All Blocks Over and Add" / "16 Ways To Win"game. Player stacks 6 numbered wooden blocks in a pyramid on the teeter board, then tosses the ball to tip them and add up the score.
